Adjusting the combined anteversion to maximize range of motion in total hip arthroplasty: Acetabular side or femoral side, which adjustment is more effective?

چکیده

INTRODUCTION: Cup and stem positioning correlates each other in total hip arthroplasty (THA), and it is believed that optimal combined anteversion (CA), which is a sum of cup and stem anteversion (AV) angle, is about 40° – 60°. However, there are few studies that have compared which adjustment is more effective, changing a cup or a femoral stem to optimizing CA and increasing ROM. In this study, we compared the ROM resulting from various CA parameters by changing AV angles of cup and stem using a THA model that permits variations in femoral AV.

A computer model of the position of the combined component in the prevention of impingement in total hip replacement.

Dislocation remains a major concern after total hip replacement, and is often attributed to malposition of the components. The optimum position for placement of the components remains uncertain. We have attempted to identify a relatively safe zone in which movement of the hip will occur without impingement, even if one component is positioned incorrectly. A three-dimensional computer model was ...

Comparison of native anatomy with recommended safe component orientation in total hip arthroplasty for primary osteoarthritis.

BACKGROUND The adverse consequences of impingement, dislocation, and implant wear have stimulated increasing interest in accurate component orientation in total hip arthroplasty and hip resurfacing. The aims of the present study were to define femoral and acetabular orientation in a cohort of patients with primary hip osteoarthritis and to determine whether the orientation of their native hip j...

Factors affecting bony impingement in hip arthroplasty.

Computer modeling of 10 patients' computed tomographic scans was used to study the variables affecting hip arthroplasty range of motion before bony impingement (ROMBI) including acetabular offset and height, femoral offset, height and anteversion, and osteophyte removal. The ROMBI was compared with the ROM before component impingement and the native hip ROM. The ROMBI decreased with decreased t...

The combined anteversion technique for acetabular component anteversion.

The combined anteversion technique for acetabular component placement of total hip arthroplasty is beneficial because of the surgeons' limited ability to control the anteversion of a cementless femoral stem. Our data show that the cementless stem anteversion can be 15 degrees different than anticipated. By determining femoral stem anteversion before positioning cup anteversion, the cup antevers...
